Building on the 1997 Unifying Theory of ADHD by Dr. Russell A. Barkley, PhD, I hereby provide an update to this seminal work, partly to honour Dr. Barkley for his 40+ years of relentless commitment to solving the ADHD enigma and partly because I wish to share some groundbreaking insights into the root causes of ADHD and ASD, based on the latest scientific research from a variety but correlated fields of study.

Introducing: The Unified Embodied Theory of ADHD

Abstract
This paper presents The Unified Embodied Theory of ADHD, a comprehensive framework moving the clinical understanding of neurodevelopmental disorders from descriptive behavioral taxonomy to mechanistic computational models.

By integrating the Free Energy Principle (Active Inference) with Russell Barkley’s Theory of Behavioral Inhibition, the author posits a circular causal chain originating in Maternal Circadian Disruption (MCD).

The theory argues that genetic “eveningness” in the mother functions as a physiological dysfunction rather than a lifestyle choice, desynchronizing the Placental Clock and exposing the fetus to inappropriate cortisol levels. This programs the fetal hypothalamic-pituitary-adrenal (HPA) axis for chronic Allostatic Load prior to birth.

Physiologically, the paper redefines executive dysfunction as a failure of Mitochondrial Allostasis—a rapidly depleting “fuel tank” that renders metabolically expensive “top-down” inhibition unsustainable.

Behaviorally, hyperactivity and fidgeting are recontextualized as “Epistemic Foraging,” a functional strategy utilized by the brain to generate reliable proprioceptive feedback (reducing entropy) in the presence of “noisy” interoception.

The framework differentiates between conditions by defining ADHD as a “Brake Failure” (low precision on inhibitory signals) and ASD as a “Manual Transmission” driven by rigid Hyper-Priors and sensory overload.

Finally, pharmacological interventions are analyzed as tools for modulating the neural Signal-to-Noise Ratio (SNR) and optimizing neural gain.
